STEP 1. ENLISTMENT AND ASSESSMENT (Students with Back Subject/s Only)

A. Junior High School

1.1 Coordinate with International School through email at cav-is@lpu.edu.ph

B. Senior High School 

1.1 Log-in to “Student Access Module” by entering the Username (Student No.), Birthdate and corresponding Password. If the user forgot the password, just click on “Forgot your password

1.2 Click the “Registration Tab”.

  • For STUDENTS WITH BACK SUBJECT/S, do not select any section from the dropdown. Just tick the subject/s one-by-one and select the desired schedule. Selected subjects and schedules will be displayed under the Registered Subjects portion.
  • Or, just click the “Available Subjects” button then tick the subjects and select the preferred schedule.

1.3 Click the “Assessment” button at the end of the page. Select preferred payment mode then click “save” button.

  • Make sure that all the selected subjects and schedules are correct before completing the  registration as changes can no longer be done.
  • Click the “printer-friendly” to generate unofficial copy of enrolment assessment form for  your payment reference.

STEP 2. PAYMENT OF FEES

Pay the assessed fees. Please see payment instructions

STEP 3. VERIFICATION OF ENROLMENT

Log-in to “Student Access Module”. Go to “Account Tab” and click the “desired semester” to view and print the “Statement of Account/Official Enrolment Assessment Form.

Shifting of Program

Undertaking Form to Change Track
1.     Fill-out the form completely and email the scanned copy together with the undertaking form for change of track:

2.     Applicable for students who want to change currently enrolled program to another.
3.     Must be applied before the student enrolls.
4.     Deadline of Application: July 20, 2021
Withdrawal of Subject1.     Fill-out the form completely and email the scanned copy:

Accomplish and submit the Online Enrollment Application Form following the Schedule of Enrollment (first-come, first-served basis). Completion of the electronic form concludes the student’s online enlistment of subjects.

STEP 2. REGISTRATION DETAILS AND ASESSMENT OF FEES

For JUNIOR HIGH, the Unofficial Enrolment Assessment Form along with the Online Enrolment Guide including payment instructions will be sent to respective email accounts within two (2) days after the registration.

For SENIOR HIGH, registration details including the Unofficial Enrolment Assessment Form will be available in the student portal within the scheduled enrolment dates.

2.1 On web browser, open the URL cavite.lpu.edu.ph to proceed with student’s log-in portal.

2.2 Search and click “Student Access Module”. Log-in to the student portal by entering the Student Number as the Username and the assigned Password. If the user forgot the password, just click on “Forgot your password? Click here”

2.3 Click “Registration” then click “Assessment” button to generate a printer-friendly version of your unofficial EAF.

STEP 3. PAYMENT OF FEES

STEP 4. ENROLMENT VERIFICATIONS
