STEP 1. ENLISTMENT AND ASSESSMENT
A. Regular Subject Offerings
Note: Allowed no. of units to be enrolled during Special Term is maximum of 9 units only.
1.1. Log-in to “Student Access Module” by entering the Username (Student No.), Birthdate and corresponding Password. If the user forgot the password, just click on “Forgot your password?
1.2 Click the “Registration Tab”.
- For REGULAR STUDENTS, select your preferred block section from the dropdown.
- For IRREGULAR STUDENTS, do not select any section from the dropdown. Just tick the subject/s one-by-one and select the desired schedule. Selected subjects and schedules will be displayed under the Registered Subjects portion or just click the “Available Subjects” button then tick the subjects and select the preferred schedule.
Note: Always check the allowable number of units.
1.3 Click the “Assessment” button at the end of the page. Select preferred payment mode then click “save” button.
- Make sure that all the selected subjects and schedules are correct before completing the registration as changes can no longer be done.
- Click the “printer-friendly” to generate unofficial copy of enrolment assessment form for your payment reference.
B. Practicum/OJT/Internship

Student | 1. Submits duly accomplished practicum/on-the job training/internship initial requirements to respective college. College | 1. Reviews and assess the submitted requirements of the student. 2. Enlists the subject to the student once initial requirements for practicum/on-the job training/internship are completed and complied. 3. Informs the student with the status of the submitted requirements/enlistment. |
Student | 1. Views the assessed fees in the student portal for reference. 2. Pays the corresponding tuition and other fees for enrolment of practicum/on-the job training/internship subject. |
Practicum/OJT/Internship Adviser/College | 1. Forwards the additional requirements for practicum/on-the job training/internship subject to the student for compliance. |
STEP 2. PAYMENT OF FEES
Pay the assessed fees. Please see payment instructions
STEP 3. VERIFICATION OF ENROLMENT
Log-in to “Student Access Module”. Go to “Account Tab” and click the “desired semester” to view and print the “Statement of Account/Official Enrolment Assessment Form.”

APPLICATION ATTACHMENTS FOR WITHDRAWAL/DROPPING OF SUBJECT:
- Letter from student’s parent or guardian stating the reason for withdrawing/dropping of subjects addressed to the Registrar’s Office along with supporting documents such as certifications or records (e.g., medical certificate for medical reasons)
- Photocopy of valid ID from parent/guardian.
- To be submitted to the Registrar’s Office for screening of application (on campus, from Monday to Thursday, 8:00 AM to 4:00pm).

HOW TO APPLY:
- Fill-out the form completely.
- Email a clear scanned copy of the form to respective college.
