  3. Original and Photocopy of Diploma
  4. Original and Photocopy of PSA Birth Certificate
  5. Original and Photocopy of Marriage Certificate (if married)
  6. Undertaking Form
  7. Complete and signed copy of terms and conditions of LPU-Cavite Data Privacy Policy (click to print)

FRESHMEN

  • Transcript of Records, with remarks: “For Further Studies” (from previous school)

TRANSFEREES

  • Transfer Credentials/Honorable Dismissal
  • Transcript of Records from previous school

ALTERNATIVE LEARNING SYSTEM (ALS)/NON-FORMAL GRADUATES

  • Two (2) pcs. 2×2 Colored Photos and Passport-Sized Photo (white background, with name and signature at the back)
  • Original and Photocopy of PSA Birth Certificate 
  • Complete and signed copy of terms and conditions of LPU-Cavite Data Privacy Policy (click to print)

  • Retention Policy for BS Accountancy and BS Customs Administration student applicants

FOREIGN STUDENTS

  2. Alien Certificate of Registration and Student Visa (18 years old & above)/SSP (below 18 years old)
  3. Photocopy of Passport (Biopage)
  4. Authenticated Scholastic Records (Red Ribbon, with English translation)
  5. Original Notarized Affidavit of Support including bank statements or notarized notice of grant for institution-al scholars to cover expenses for the student’s accommodation and subsistence, as well as school dues and other incidental expenses.
  6. Medical Certificate/Quarantine Medical Clearance (if in the Philippines)
  7. Original Certificate of Course Description and Certification of Grades (for transferee)
  8. Transfer Credentials/Honorable Dismissal (for transferee)
  9. Certificate of Good Moral Character (if applicable)
  10. Original Birth Certificate or its equivalent duly authenticated by the Philippine Foreign Service Post
  11. Additional forms available at the Admissions Office: Undertaking Form and Data Privacy Policy

– All applications are subject for review and approval.

Instructions:   Print and fill-out the form completely. Proceed to respective units/department/colleges concerned for interview and signature. Please note that only those fully accomplished form will be accepted for processing.
