ONLINE REGISTRATION AND ENROLLMENT PROCEDURES

STEP 1. REGISTRATION AND ASSESSMENT

  1. Log-in to “Student Access Module” by entering the Username (Student No.), Birthdate and corresponding Password. If the user forgot the password, just click on “Forgot your password?
  2. Click the “Registration Tab”. 
    • For REGULAR STUDENTS, select your preferred block section from the dropdown.
    • For IRREGULAR STUDENTS, do not select any section from the dropdown. Just tick the subject/s one-by-one and select the desired schedule. Selected subjects and schedules will be displayed under the Registered Subjects portion.
    • For STUDENTS WITH BACK SUBJECT/S, just click the “Available Subjects” button then tick the subjects and select the preferred schedule.
    • Note: Always check the allowable number of units.
  3. Click the “Assessment” button at the end of the page. Select preferred payment mode then click “save” button.
    • Make sure that all the selected subjects and schedules are correct before completing the registration as changes can no longer be done.
    • Click “printer-friendly” to generate unofficial copy of enrolment assessment form for your payment reference.

STEP 2. PAYMENT OF FEES

Pay the assessed fees. Please see payment instructions

STEP 3. ENROLMENT VERIFICATION

Log-in to “Student Access Module”. Go to “Account Tab” and click the “First Semester AY 2023-2024” to view and print the “Statement of Account/Official Enrolment Assessment Form”.

Please note that REGISTERED SUBJECTS in the REGISTRATION TAB will no longer be available once officially enrolled. Subjects enrolled may be viewed by clicking the EAF in the Account Tab.

APPLICATION ATTACHMENTS:
– Letter from student’s parent or guardian stating the reason for withdrawal or dropping of subjects addressed to the Registrar’s Office along with supporting documents such as certifications or records (e.g., medical certificate for medical reasons)
– Photocopy of a valid ID from a parent or guardian
– To be submitted to the Registrar’s Office for screening of application.
(Monday to Friday, 8:00 AM to 4:00 PM)
PERMIT TO SHIFT PROGRAM
– Applicable to students who wants to shift from one program to another.
– Must be cleared of all accountabilities.
– Must be applied BEFORE the student enrolls.

– Applicable to returning students who stopped for at least one semester or semester.
– Must be applied BEFORE the student enrolls.

SUBSTITUTION OF SUBJECT/S
– Applicable if the subject is no longer offered at LPU, or if the subject is not offered during a particular semester and the student is graduating.
– Must be applied BEFORE the student enrolls.

OVERLOAD OF SUBJECT/S
– Applicable only to graduating students.
– Overloading shall be allowed up to a maximum of six (6) units.
– Must be applied BEFORE the student enrolls.

ADDING/CHANGING OF SUBJECT/S
– Applicable for students who wants to add subject based on the allowable units required in the curriculum or change a particular subject/schedule for conflict/dissolved classes.

INSTRUCTIONS
1. Print and fill-out the form completely.
2. Proceed to respective units/department/colleges concerned for interview and signature.
3. Please note that only those fully accomplished form will be accepted for processing.
